New patient forms - Downloads
Most new patient want to know how to get started with the process of getting their new braces. Here is some information that will help you save time and know what to expect during your initial visit to our office :)
- First, you will fill out paperwork so we can clearly assess your dental needs please see pdf icon below to download and print forms. Or, our office can e-mail you the forms to be downloaded and printed before your appointment to help you save time.
- When you visit our office a scheduling coordinators will help you check-in using our automated check-in screen. They will then notify our Treatment Coordinator, that you have checked in.
- Next, we will take a digital full mouth x-ray (Panorex) before you are escorted to one of our consultation rooms where you will meet Dr. Frost.
- We will take photos of your teeth, facials and profile. These are tools that help Dr. Frost understand your overall dental health and past dental history.
- The doctor will conduct a thorough examination and discuss your treatment options. We want to make sure you are comfortable in our office and we will spend as much time as you need to have your questions answered.
You should allow 1 hour for your initial exam. If you are under 18 years of age we will need a parent or guardian with you at the exam. We can schedule an appointment to bond your Damon braces after your consultation. We can even schedule you that same day!
